Clinical Analysis of Perforated Typhoid Ulcer

Abstract
This report represents a clinical study of 52 cases of ileal perforation due to typhoid fever. These cases were treated by surgery in department of surgery of Wonju Christian Hospital. The period covered is from March 1963 to February 1976, about 13 years. The result is summarized as follows;
1) Sex ratio between male and female was 2.9: 1
2) Salmonellosis occurred in most age groups but appeared most frequently between the ages of 16-30 year.
3) The occurrence of salmonellosis was most frequent in summer.
4) Perforation was most frequent at 6 th to 20 th day after onset.
5) Widal test was positive in 51.3% (above 160X) and no agglutination in 30%. 6) WBC count was normal or decreased in 58%.
7) Subdiaphragmatic free air in upright abdominal X-ray was found in 60% paralytic ileus sign in 90%.
8) The site of intestinal perforation was, within 60cm proximal from ileocecal junction in 77% and most frequent within 10cm to 20cm.
9) Number of perforation was single in 77% and multiple in 23%.
10) In surgical management simple closure was done in 25%, end to end anastomisis after resection in 21%, and exterorization in 54%.
11) Complications after surgery were;
Wound infection: 56%
Intraabdominal abscess: 10% Pneumonia: 6%
Fecal fistula: 6%
12) Overall mortality was 17.3%.
